REGISTRATION PERIOD: There will be one winner drawn per drawing collection location during the registration period.

All requested information on the entry form must be completed to enter. By entering the Contest, all Entrants agree to be bound by the Official Rules of this Drawing. No Entries will be acknowledged or returned to the Entrant under any circumstance. PRIZE WINNER NOTIFICATION: The official Georgia Chiropractic Group at Sixes contact(s) representing the winners will be notified by telephone, email or social media provided by the Entrant and requested to respond within 24 hours. If the finalists do not respond within 24 hours then another winner is selected.
GENERAL PRIZE TERMS: Odds of winning will depend on the number of entries. All costs and expenses not specifically listed above, including, without limitation, sale or use tax, shipping and handling charges to be incurred in connection with redemption of the Prize are solely Winner’s responsibility. Upon Entry into the Contest, Winner (as well as all other Entrants) are required to comply with any and all applicable federal, state and local laws, rules and regulations.
